I did lots of split test & tweaked for months to get my best converting page & I suggest you do the same.
Its pretty much easy to get a good converting page if you can make the page simple & straight forward.
I don’t use double opt-in & don’t ask for name & it works for me. Increased my conversion significantly.
Another thing, I noticed social proof can impact on your conversion rate a lot so put a facebook comment box.
